Clinical Field Specialist – Mobile Role (Plenty of Driving)

Location: Northern California (Skilled Nursing Facilities)

Travel: Daily driving required (mileage reimbursement provided)

As a Clinical Field Specialist , you will be a trusted on-site presence supporting patient care, conducting cognitive and clinical assessments, and ensuring seamless communication between patients, facility staff, and our clinical team.

This is a highly mobile, field-based role designed for someone who is reliable, proactive, and energized by working independently while making a meaningful impact.

What You’ll Do:

• Travel daily to multiple skilled nursing facilities (2–3 sites per day).
• Conduct cognitive and clinical assessments with senior patients.
• Communicate findings clearly to our provider team to support care planning.
• Serve as a consistent, trusted presence for patients and facility staff.
• Build strong relationships across multiple facilities and care teams.
• Use mobile tools and EHR systems to document visits and manage follow-ups.
• Identify gaps in care, solve problems in real time, and ensure follow-through.
• Maintain a high level of organization and accountability while working independently in the field.

What Makes You a Strong Fit:

• Clinical background (CNA, MA, or similar) with 2+ years of patient-facing experience.
• Experience working with geriatric or skilled nursing populations strongly preferred.
• Comfortable with significant daily driving and managing a mobile schedule.
• Highly dependable, punctual, and able to work autonomously.
• Strong work ethic with a natural sense of ownership and follow-through.
• Excellent communicator who can quickly build trust with patients and care teams.
• Tech-savvy and comfortable using EHRs and mobile tools in a fast-paced environment.

Travel & Compensation:

• This is a field-based role requiring daily driving between facilities.
• Candidates must have a reliable vehicle, and a valid driver’s license.
• Mileage reimbursement provided.
• Role is structured for efficient regional travel and route optimization.
